Monte Cook Games is headed back to the backwoods with a brand new adventure anthology for their Old Gods of Appalachia RPG. Entitled Come Hell or High Water, the book contains three adventures, plus new standalone locations, lore and GM advice on building out a complete campaign.

What is Come Hell or High Water about?
Come Hell or High Water is a 128-page adventure anthology for the Old Gods of Appalachia RPG. As you might expect, it’s geared entirely for GMs, so you’ll definitely want to keep players away from it if you’re planning on running any of the material. The book is written by Shanna Germain (who has worked on a number of OGoA products) and includes:
- Three full-length adventures: This includes a traveling freakshow, a demonic creature hiding in plain sight and an unexpected arrival that threatens the lives and sanity of the adventurers.
- New GM advice: This consists of a number of tips on how to building your own adventures and campaigns (although it’s general enough that it can be applied to just about any horror TTRPG)
- Sites from the show: The book features a number of sites pulled from episodes of the original Old Gods of Appalachia podcast, including the Walker House (a cursed hunting lodge in Tennessee) and the Clutch (a holler of mysterious women).
- New locations: These can be dropped into the adventures or into an existing campaign and include a rundown factory, a haunted wood, a cursed lake and a forgotten hotel.

What is the Old Gods of Appalachia RPG?
Old Gods of Appalachia: The Roleplaying Game takes place in an alternate version of the Appalachian Mountains and blends cosmic horror with regional folklore. It’s a setting where abandoned coal towns, mist-shrouded hollers and forgotten mines hide ancient, sleeping powers.
Like other popular horror TTRPGs such as Call of Cthulhu, players take on the roles of ordinary people who are taken out of their everyday lives to confront dark and mysterious beings that exist in the American wilderness.

Notably, the game runs on Monte Cook Games’ Cypher System; a rules-light engine that emphasizes narrative storytelling and player-driven choices. Its mechanics are designed to build tension, with players building characters through a simple sentence: “I am an [Adjective] [Noun] who [Verbs]
The Old Gods of Appalachia RPG was first announced in 2021 as a collaboration between Monte Cook Games and Steve Shell and Cam Collins, the creators of the award-winning Old Gods of Appalachia horror podcast. In early 2022, the team launched a Kickstarter campaign, which raised over $2 million USD and became one of the most successful tabletop RPG campaigns at the time. The core rulebook was later released in 2023 by Monte Cook Games and the line has since expanded with several new supplements, adventures and digital tools.

Why we’re excited about Come Hell or High Water?
- Lore from the show: With over 150 episodes to draw on, the Old Gods of Appalachia podcast has a ton of lore, so it’s great to see the TTRPG version tap into this material.
- New adventures: As a relatively young game, we haven’t seen a ton of adventures, so GMs looking to expand their campaign or take a first crack at the system will find a lot to love here.
- A different kind of horror: OGoA does a fantastic job of exploring the darker side of America history, where poverty and racism are often just as terrifying as the monsters that lurk in the shadows. It’s something that really sets the game apart from more well-known systems like Call of Cthulhu, and we’re excited to see how these new adventures explore those themes.
- The creative team: This new anthology features contributions from Steve Shell and Cam Collins, creators of the original OGoA podcast, so it’ll be interesting to see just what they can bring to the table.

Final Thoughts
Old Gods of Appalachia has definitely managed to make a mark on the TTRPG horror scene, so it’s great to see the game continuing to be supported with new content. Whether these adventures will scare up any new fans, however, remains to be seen.
